Offered to Prairie View Lodge Independent Living Suite Tenants
Prairie View Lodge (PVL) Board of Directors is excited to offer additional Assisted Living Services to those tenants living in the Prairie View Lodge Independent Living Suites. While there is always the option to obtain meals from the kitchen at present, we have had requests to add even more additional services.
The Prairie View Lodge Assisted Living Program will include 2 meals a day and snacks, weekly light housekeeping and laundry services. This package has been offered as an option to everyone who is currently a tenant, as well as to all new tenants. If a tenant declines the package and finds at a later time that they would like to take advantage of the services, they can sign up at any time. The cost of the package would then be added to their suite rent. The service is designed to provide the services so that our tenants can “Age in Place”; remaining in their home comfortably.
As is currently the practice, there are no personal care services provided to the Independent Living Suite tenants by the Prairie View Lodge staff; however there are some social opportunities. The Tenant Wellness Coordinator works three days a week and offers programs including games, coffee gatherings, exercise groups and 1 on 1 visits. There is also a variety of other entertainment as well as the weekly Church Services held at Prairie View Lodge.

To reach help urgently for any age call the Mental Health Crisis Line at 1-888-588-1697 or 1-866-588-1697, they will assist with identifying resources or present to the nearest hospital Emergency Department.
Referral forms for in person consultation are available on the Southern Health-Santé Sud website by searching Mental Health and selecting applicable referral form Seniors, Adult or Child and Adolescent, referral forms can be also requested by calling 1-888-310-4593. A community Bath Program is available to those in need of bathing services. The program is available from both the Rock Lake Hospital and the Rock Lake Personal Care Home.
The cost of each bath is $10.00. For applications, please contact the HomeCare office at (204) 873 – 3030.
A Clinical Dietician is available to provide nutrition services. For an appointment, please contact the Rock Lake Clinic at (204) 873 – 2424.
Home Care in the Southern Health – Sante Sud Region is a comprehensive community-based program that provides essential in-home care support to individuals, regardless of age, who require health services or assistance with activities of daily living. This program augments the resources of family and community, using a model in which emphasizes promotion of care in the home, facilitation of hospital discharge and provision of alternatives to entry into the long term care system for as long as it is safely possible.
Services include: Assessment of eligibility, care planning, case management, nursing services, therapy services, health teaching, personal care, meal preparation, respite/family relief, cleaning and laundry, access to Adult Day Care, assessment for the facilitation of personal care home placement and medical equipment and supplies to support the care plan.
Home Care offices are located at Rock Lake Hospital.

Help is just a push away; one press of your lightweight help button activates a small in-home unit. It puts you in touch with a caring lifeline monitor who talks to you to find out what kind of help is needed. They will then call one of your responders, a friend or a neighbour, who will come to your home to check up on you.
The Lifeline Program is managed and operated out of the Rock Lake Hospital.

Louise Community Support Services offers a resource service to seniors throughout the Municipality of Louise with office hours in Pilot Mound (204) 825 – 2443 Monday, Thursday and Friday, and in Crystal City (204) 873 – 2103 on Tuesday. For more information on services available please call the coordinator.
The Meals on Wheels service delivers meals to Seniors in both Pilot Mound and Crystal City. The service is available seven days a week thanks to the countless volunteers who drive and deliver the meals for the program. Volunteers from the Hospital to Crystal City citizens and from Rock Lake Person Care Home to Pilot Mound citizens deliver meals. It is the evening meal that is delivered.
Cost of the program is $7.00/meal, and will be billed on a monthly basis.

Occupational Therapy services are available through Rehabilitation Services, Boundary Trails Health Centre. Visits are made to Rock Lake Personal Care Home and Prairie View lodge one day every two weeks.
Appointments can be made through the Administration Office.
Palliative Care is active, compassionate care and support for the chronic and terminally ill at the time when there is no cure for the disease. The focus is on improving the person’s quality of life.
Palliative care services are offered by a group of dedicated professionals and volunteers who seek to provide physical, emotional and spiritual support to individuals and their families at home, in long term care facilities or in the hospital.
Palliative Care is about… Compassion, Community, Dignity, Choice, Comfort, Life, Support, and Hope and Finding Meaning.

Rock Lake Hospital Palliative Care:
A dedicated, Accredited Level 2 Paramedic Ambulance Service providing advanced life support to the community 24 hours a day, seven days a week. The service is operated out of the Rock Lake Hospital. Program Coordinator is Jeff Nedohin, (204) 873 – 2132 ext. 6.
Rock Lake Handi-Van service is managed and operated by the Municipality of Louise. The service is available to all residents within the Municipality of Louise. Community residents are fortunate to have reliable, caring drivers who provide great assistance as needed. The fee for this service is $20.00 per hour within the Municipality of Louise, with an added $0.50 cents per km outside the Municipality.

The SSGL program is designed to assist the tenants at Prairie View Lodge Independent Living Suites in accomplishing their life management skills. Services provided by the Tenant Wellness Coordinator 3 days per week, will be made available in accordance with individual assessed needs. The program is meant to enhance the quality of life for the tenants and is not intended to duplicate those services already provided such as Home Care.
The Tenant Wellness Coordinator will work with the tenants, determining their needs for support in their Activities of Daily Living, enabling them to remain independent within the community. These activities may include but are not limited to:
- Placing grocery orders
- Navigating the phone system to assist in making appointments
- Arranging transportation
- Assist in making arrangements for getting to and/or appointments
- Involving tenants in planning activities to facilitate their participation in decision making processes and assessing individual needs
- Promoting group activities that facilitate group interactions between clients and/or individual attention as required
- Plan social activities that are relevant to the seasons of the year, to current events or to cultural and religious holiday
